In Memory Application

To have a loved one considered for the In Memory program in 2012, you must submit your application to VVMF by April 12th, 2012.  Please fill out the information below.  Along with the application, please upload copies of the death certificate and military records showing service in Vietnam (ie: DD214) as well as two clear photographs of the honoree.
